Miwatj Health Aboriginal Corporation is the regional Aboriginal Community Controlled Health Service in East Arnhem Land, providing comprehensive primary health care services for over 6,000 Indigenous residents of North East Arnhem and public health services for close to 10,000 people across the region.

You will provide high quality Chronic Disease Health management by working with AHP’s and Community Workers. You will need to support and maintain the Chronic Disease register/recall database; provide advocacy and support for chronic disease self-management and ensure all chronic disease clients have a current GP Management Plan and Team Care Arrangement.
You will need a minimum of 3 years’ experience working in remote Aboriginal communities and a strong interest in the health issues impacting on Aboriginal people in northern Australia.
